Diabetes Management

Diabetes mellitus is one of the most prevalent chronic conditions affecting millions of Indians. At Divine Healthcare, Dr. Singh provides personalized, evidence-based diabetes care that goes beyond just controlling blood sugar — focusing on preventing complications and improving quality of life.

Diabetes blood sugar testing

Understanding Diabetes

Diabetes is a chronic metabolic disorder characterized by elevated blood glucose levels resulting from defects in insulin secretion, insulin action, or both. India is often called the "Diabetes Capital of the World," with over 77 million adults living with the condition.

There are three primary types:

  • Type 1 Diabetes: An autoimmune condition where the body's immune system attacks insulin-producing beta cells in the pancreas. Typically diagnosed in children and young adults, it requires lifelong insulin therapy.
  • Type 2 Diabetes: The most common form, accounting for over 90% of cases. It develops when the body becomes resistant to insulin or the pancreas doesn't produce enough insulin. Strongly linked to obesity, sedentary lifestyle, and genetic predisposition.
  • Gestational Diabetes: Develops during pregnancy and usually resolves after delivery, but significantly increases the risk of developing Type 2 diabetes later in life.

Warning Signs & Symptoms

Frequent urination (Polyuria)
Excessive thirst (Polydipsia)
Increased hunger (Polyphagia)
Unexplained weight loss
Chronic fatigue & weakness
Blurred vision
Slow-healing wounds
Tingling or numbness in hands/feet

Our Treatment Approach

Dr. Singh's diabetes management protocol is comprehensive and individualized:

1
Thorough Assessment

Complete metabolic panel including Fasting Blood Sugar, Post-Prandial Blood Sugar, HbA1c (glycated hemoglobin), lipid profile, kidney function tests, and urine microalbumin screening.

2
Personalized Treatment Plan

Based on the type and severity of diabetes, we create a customized plan combining oral hypoglycemic agents, insulin therapy (if needed), and lifestyle modifications tailored to the patient's daily routine.

3
Complication Prevention

Regular screening for diabetic retinopathy, nephropathy, neuropathy, and cardiovascular disease. Target Organ Damage (TOD) prevention is our highest priority.

4
Lifestyle & Diet Counselling

Practical dietary guidance suitable for Indian diets, exercise recommendations, stress management, and patient education for self-monitoring of blood glucose.

Did You Know?

Pre-diabetes (Impaired Glucose Tolerance) affects nearly 136 million Indians. With early intervention through lifestyle changes and medication, progression to full diabetes can be prevented or delayed significantly. Regular screening is recommended for all adults above 30, especially those with a family history of diabetes.

Hypertension & Heart Care

High blood pressure is a silent killer that damages your heart, brain, kidneys, and blood vessels over years without obvious symptoms. Dr. Singh specializes in aggressive yet personalized blood pressure management to prevent life-threatening complications.

Blood pressure monitoring

Understanding Hypertension

Hypertension (high blood pressure) occurs when the force of blood against your artery walls is consistently too high. A normal blood pressure reading is below 120/80 mmHg. When readings consistently exceed 130/80 mmHg, it is classified as hypertension.

Hypertension is classified into stages:

  • Elevated Blood Pressure: Systolic 120–129 and diastolic less than 80 mmHg. Lifestyle changes are recommended.
  • Stage 1 Hypertension: Systolic 130–139 or diastolic 80–89 mmHg. Medication may be needed alongside lifestyle changes.
  • Stage 2 Hypertension: Systolic 140+ or diastolic 90+ mmHg. Combination medication therapy is usually required.
  • Hypertensive Crisis: Systolic over 180 and/or diastolic over 120 mmHg. This is a medical emergency requiring immediate care.

Complications of Uncontrolled BP

Heart Attack & Heart Failure

Chronic high BP forces the heart to work harder, causing the heart muscle to thicken and eventually weaken, leading to heart failure or acute myocardial infarction.

Stroke

Hypertension is the single most important risk factor for stroke. It damages blood vessels in the brain, increasing the risk of both ischemic and hemorrhagic strokes.

Kidney Damage

High blood pressure damages the delicate filtering units of the kidneys, potentially leading to chronic kidney disease and the need for dialysis.

Vision Loss

Hypertensive retinopathy damages the blood vessels in the retina, potentially causing blurred vision, bleeding, and even permanent vision loss.

Our Treatment Approach

At Divine Healthcare, we take a systematic approach to blood pressure management:

  • Accurate Diagnosis: Multiple blood pressure readings across visits, 24-hour ambulatory monitoring if needed, and ruling out secondary causes of hypertension.
  • Medication Management: Evidence-based selection of antihypertensive drugs (ACE inhibitors, ARBs, calcium channel blockers, diuretics) based on the patient's profile, comorbidities, and response.
  • Lifestyle Modifications: DASH diet recommendations, sodium restriction, regular physical activity, weight management, alcohol limitation, and stress reduction techniques.
  • Regular Monitoring: Scheduled follow-ups with blood pressure tracking, kidney function tests, ECG evaluation, and cardiovascular risk assessment.

In-House ECG Facility

Divine Healthcare is equipped with a modern Electrocardiogram (ECG) machine for immediate cardiac evaluation. No need to visit a separate diagnostic centre — get your heart tested right here at the clinic.

ECG Heart Monitoring Equipment

What is an ECG?

An Electrocardiogram (ECG or EKG) is a simple, painless test that records the electrical activity of the heart. Each heartbeat is triggered by an electrical impulse generated from special cells in the upper right chamber of the heart. An ECG records these impulses to show how fast the heart is beating, the rhythm (steady or irregular), and the strength and timing of the electrical signals as they pass through each part of the heart.

The test involves placing 10 small electrode patches on the chest, arms, and legs. These electrodes detect the tiny electrical changes on the skin that arise from the heart muscle's electrophysiologic pattern of depolarizing and repolarizing during each heartbeat. The entire procedure takes less than 5 minutes and is completely painless.

When Do You Need an ECG?

Dr. Singh may recommend an ECG if you experience any of the following:

Chest pain or tightness
Heart palpitations or racing heartbeat
Dizziness or lightheadedness
Shortness of breath
Unexplained fatigue
Fainting or near-fainting episodes

ECG is also recommended as a routine screening tool for patients with diabetes, hypertension, high cholesterol, or a family history of heart disease.

What Can an ECG Detect?

  • Heart Attack (Myocardial Infarction): Both current and past heart attacks leave distinctive patterns on the ECG.
  • Arrhythmias: Abnormal heart rhythms including atrial fibrillation, tachycardia, bradycardia, and heart blocks.
  • Heart Chamber Enlargement: Signs of the heart working harder than normal, which may indicate underlying conditions.
  • Electrolyte Imbalances: Abnormal levels of potassium, calcium, and other minerals that affect heart function.
  • Coronary Artery Disease: Reduced blood flow to the heart muscle can show characteristic ECG changes.

Thyroid

Thyroid Disorders

Hypothyroidism, Hyperthyroidism, Goitre & Thyroid Nodules

Understanding Thyroid Disease

The thyroid is a small, butterfly-shaped gland located at the base of your neck. Despite its small size, it produces hormones (T3 and T4) that regulate nearly every metabolic process in your body — from heart rate and body temperature to energy levels and weight management.

Hypothyroidism (Underactive Thyroid)

When the thyroid produces too little hormone, the body's processes slow down. This is the most common thyroid disorder, especially among women. Symptoms include unexplained weight gain, fatigue, cold intolerance, constipation, dry skin, hair loss, depression, and irregular menstrual cycles. Hashimoto's thyroiditis is the most common cause.

Hyperthyroidism (Overactive Thyroid)

An overactive thyroid produces excess hormones, causing the body to speed up. Symptoms include unexpected weight loss, rapid heartbeat, anxiety, tremors, excessive sweating, heat intolerance, and frequent bowel movements. Graves' disease is the most common cause.

Our Treatment Approach

Dr. Singh performs thorough thyroid function testing including TSH, Free T3, Free T4, and thyroid antibodies when indicated. Treatment focuses on achieving hormonal balance through medication (Levothyroxine for hypothyroidism, anti-thyroid drugs for hyperthyroidism) with regular monitoring and dose adjustments. We also address associated conditions like high cholesterol, osteoporosis, and cardiac issues that frequently accompany thyroid disorders.

Gastro

Gastrointestinal & Liver Disorders

Acid Reflux (GERD), Gastritis, IBS, Fatty Liver, Hepatitis

Common Digestive Problems

Gastrointestinal disorders are among the most common reasons for clinic visits. They range from mild, occasional discomfort to chronic conditions that significantly impact quality of life. Many digestive issues are closely linked to diet, stress, and lifestyle habits prevalent in modern Indian life.

Acid Reflux & GERD

Gastroesophageal Reflux Disease occurs when stomach acid frequently flows back into the oesophagus. Chronic GERD can lead to oesophagitis, Barrett's oesophagus, and even oesophageal cancer if left untreated. Symptoms include heartburn, regurgitation, chest pain, difficulty swallowing, and chronic cough.

Irritable Bowel Syndrome (IBS)

IBS is a functional disorder affecting the large intestine, causing cramping, abdominal pain, bloating, gas, diarrhoea, and constipation. While not life-threatening, it significantly affects daily life and requires long-term management through dietary modifications, stress management, and targeted medications.

Non-Alcoholic Fatty Liver Disease (NAFLD)

NAFLD has become an epidemic in India, closely associated with obesity, diabetes, and metabolic syndrome. Fat accumulation in the liver can progress to inflammation (NASH), fibrosis, cirrhosis, and liver failure. Early detection through liver function tests and ultrasound is crucial for reversing the condition.

Our Treatment Approach

Dr. Singh identifies dietary triggers, screens for H. pylori infection (a common cause of gastritis and ulcers), monitors liver function, and prescribes evidence-based treatments. We emphasize dietary counselling tailored to Indian food habits, lifestyle modifications, and regular follow-up to prevent progression of liver disease.

Fever

Fever & Seasonal Infections

Viral Fever, Dengue, Typhoid, Malaria, COVID, Respiratory Infections

Managing Infectious Diseases

Febrile illnesses are extremely common in India, with seasonal outbreaks of dengue, malaria, typhoid, and influenza. Accurate clinical diagnosis is essential to differentiate between viral and bacterial infections, ensuring appropriate treatment and avoiding unnecessary antibiotics that contribute to antimicrobial resistance.

Dengue Fever

Transmitted by Aedes mosquitoes, dengue causes high fever, severe headache, pain behind the eyes, muscle and joint pain, and rash. Severe dengue can lead to hemorrhagic complications and shock. Serial platelet monitoring, adequate hydration, and supportive care are the mainstays of management.

Typhoid Fever

Caused by Salmonella typhi bacteria through contaminated food and water, typhoid presents with sustained high fever, headache, abdominal pain, and constipation or diarrhoea. Appropriate antibiotic selection based on sensitivity patterns is crucial, as drug-resistant typhoid is increasingly common in India.

Respiratory Infections

Upper and lower respiratory tract infections including common cold, influenza, pharyngitis, bronchitis, and pneumonia are managed with evidence-based protocols. We differentiate between viral and bacterial causes to ensure rational use of antibiotics and appropriate supportive care.

Our Treatment Approach

Dr. Singh prioritizes clinical diagnostics with appropriate laboratory investigations (CBC, blood cultures, rapid antigen tests) to arrive at an accurate diagnosis. Treatment protocols emphasize rational antibiotic usage, adequate hydration, symptomatic relief, and monitoring for complications. Patient education on prevention, hygiene, and when to seek emergency care is an integral part of our approach.

Respiratory

Respiratory & Lung Disorders

Asthma, COPD, Chronic Cough, Bronchitis, Pneumonia

Breathing Easy, Living Better

Respiratory conditions are among the leading causes of morbidity and mortality worldwide. Air pollution in the NCR region makes respiratory health even more critical for Greater Noida residents. Dr. Singh provides comprehensive evaluation and management of both acute and chronic respiratory conditions.

Asthma

Asthma is a chronic inflammatory disease of the airways that causes recurrent episodes of wheezing, breathlessness, chest tightness, and coughing. Triggers include allergens (dust, pollen, pet dander), air pollution, exercise, cold air, and respiratory infections. Modern asthma management focuses on controller medications (inhaled corticosteroids) to reduce inflammation and reliever medications (bronchodilators) for acute symptoms.

Chronic Obstructive Pulmonary Disease (COPD)

COPD is a progressive lung disease that makes breathing difficult. It includes chronic bronchitis and emphysema, most commonly caused by long-term exposure to cigarette smoke or air pollution. Symptoms worsen over time and include chronic cough, sputum production, and progressive breathlessness. While irreversible, proper management can significantly slow progression and improve quality of life.

Our Treatment Approach

We focus on accurate diagnosis through clinical assessment and spirometry referral, identification and avoidance of triggers, optimized inhaler technique education, medication titration, vaccination against influenza and pneumococcal disease, and pulmonary rehabilitation guidance. Our goal is to minimize exacerbations and maximize lung function for every patient.

Headache

Headache & Migraine

Tension Headache, Chronic Migraine, Cluster Headache, Vertigo

Beyond Simple Pain Relief

Headaches are one of the most common neurological symptoms, but they vary significantly in type, cause, and severity. While most headaches are benign, some can indicate serious underlying conditions. Dr. Singh performs careful evaluation to determine the type and cause, moving patients from reactive pain relief to preventive management.

Tension-Type Headaches

The most common form, characterized by a dull, aching sensation all over the head, often described as a band-like pressure. Typically triggered by stress, poor posture, eye strain, and sleep deprivation. Management includes identifying triggers, stress management techniques, postural correction, and appropriate analgesics.

Migraines

Migraines are moderate to severe headaches, usually unilateral and throbbing, often accompanied by nausea, vomiting, and extreme sensitivity to light and sound. Some patients experience visual disturbances called "aura" before the headache. Migraines can be debilitating, lasting hours to days. Modern treatment includes both acute (triptans, analgesics) and preventive medications (beta-blockers, antidepressants, anticonvulsants) to reduce frequency and severity.

Our Treatment Approach

We use comprehensive trigger-mapping through headache diaries, evidence-based preventive medications, lifestyle optimization (sleep hygiene, regular meals, hydration, exercise), and appropriate acute treatment. We also screen for secondary causes including hypertension, medication overuse, and sinusitis to ensure nothing serious is missed.

Headache treatment
Arthritis

Joint Pain & Arthritis

Rheumatoid Arthritis, Osteoarthritis, Gout, Musculoskeletal Pain

Managing Joint Health

Joint pain and arthritis are extremely common, especially in India where a combination of genetic factors, dietary habits, and vitamin D deficiency contribute to a high prevalence. Dr. Singh evaluates joint complaints systematically to determine the type and cause, ensuring appropriate treatment.

Osteoarthritis

The most common form, caused by wear and tear of joint cartilage over time. Primarily affects weight-bearing joints (knees, hips) and small joints of the hands. Symptoms include joint pain, stiffness, swelling, and reduced range of motion. Management includes weight management, physiotherapy, pain relief, and joint-preserving strategies.

Rheumatoid Arthritis

An autoimmune condition where the body's immune system attacks the joint lining, causing inflammation, pain, and eventual joint destruction. Early diagnosis and aggressive treatment with disease-modifying drugs (DMARDs) can prevent permanent joint damage.

Gout

Caused by elevated uric acid levels leading to crystal deposition in joints. Presents with sudden, severe attacks of pain, redness, and swelling, typically in the big toe. Management includes acute attack treatment, uric acid-lowering therapy, dietary modifications, and adequate hydration.

Our Treatment Approach

We perform thorough clinical and laboratory evaluation (ESR, CRP, RF, Anti-CCP, Uric acid, Vitamin D), differentiate between inflammatory and degenerative conditions, initiate appropriate pharmacological therapy, and emphasize lifestyle modifications including weight management and exercise.

Elderly

Geriatric (Elderly) Care

Multi-Organ Disease Management, Polypharmacy Review, Falls Prevention

Caring for Our Elders

Geriatric medicine focuses on the unique healthcare needs of elderly patients, who often present with multiple concurrent conditions, take numerous medications, and face age-related physiological changes that require specialized attention. Dr. Singh provides compassionate, comprehensive care for senior citizens.

Polypharmacy Management

Many elderly patients take 5 or more medications simultaneously, increasing the risk of drug interactions, side effects, and adverse events. Dr. Singh carefully reviews all medications to eliminate unnecessary drugs, adjust dosages for age-related changes in metabolism, and ensure safe combinations — a process called medication reconciliation.

Common Geriatric Concerns

  • Chronic Disease Management: Coordinated care for diabetes, hypertension, heart disease, kidney disease, and other conditions that often coexist in elderly patients.
  • Weakness & Mobility: Evaluation of sarcopenia (muscle loss), vitamin deficiencies (B12, D), anaemia, and thyroid disorders that contribute to fatigue and weakness.
  • Cognitive Health: Screening for early signs of memory decline, and managing conditions that can affect cognitive function.
  • Falls Prevention: Identifying medications, vision problems, and balance issues that increase fall risk in the elderly.
  • Nutritional Assessment: Ensuring adequate nutrition despite reduced appetite, dental problems, and swallowing difficulties common in old age.

Our Treatment Approach

We provide holistic geriatric assessments covering physical, mental, and functional health. Treatment plans are gentle yet effective, with careful medication selection considering age-related changes. We involve family members in care decisions and focus on maintaining independence and quality of life.

Elderly care
Fatigue

Weakness & Chronic Fatigue

Anaemia, Vitamin Deficiency, Chronic Fatigue Syndrome, General Debility

Finding the Root Cause

Persistent weakness and fatigue are among the most common complaints in clinical practice. While often dismissed as "just tiredness," chronic fatigue frequently indicates underlying medical conditions that require proper diagnosis and treatment.

Anaemia

Iron deficiency anaemia is extremely common in India, affecting women disproportionately. Symptoms include fatigue, pallor, breathlessness on exertion, rapid heartbeat, and poor concentration. Other types include Vitamin B12 deficiency anaemia (common in vegetarians) and anaemia of chronic disease.

Vitamin D Deficiency

Despite abundant sunshine, Vitamin D deficiency is epidemic in India due to limited sun exposure, dark skin pigmentation, and dietary factors. It causes fatigue, bone pain, muscle weakness, mood changes, and increases susceptibility to infections. Supplementation under medical guidance can dramatically improve symptoms.

Other Causes We Evaluate

  • Hypothyroidism — underactive thyroid slowing metabolism
  • Uncontrolled diabetes — fluctuating blood sugar causing energy crashes
  • Chronic kidney disease — toxin buildup causing fatigue
  • Depression and anxiety — mental health conditions manifesting as physical tiredness
  • Sleep disorders — poor sleep quality due to various causes
  • Vitamin B12 deficiency — especially common in vegetarians

Our Treatment Approach

We conduct a systematic evaluation including complete blood count, iron studies, Vitamin B12, Vitamin D, thyroid function, blood sugar, kidney function, and liver function tests. Treatment targets the underlying cause rather than merely masking symptoms, ensuring lasting improvement in energy and vitality.

Kidney

Kidney & Urinary Health

Chronic Kidney Disease, UTI, Kidney Stones, Diabetic Nephropathy

Protecting Your Kidneys

The kidneys are vital organs that filter waste from the blood, regulate fluid balance, control blood pressure, and produce essential hormones. Kidney disease often develops silently over years, and early detection is key to preventing irreversible damage and the need for dialysis.

Chronic Kidney Disease (CKD)

India has a rapidly growing burden of CKD, primarily driven by diabetes and hypertension. CKD progresses through five stages, and early stages are often asymptomatic. Regular screening through serum creatinine, eGFR, and urine tests is essential for at-risk patients. When detected early, progression can be significantly slowed.

Urinary Tract Infections (UTIs)

UTIs are common bacterial infections affecting the urinary system. Women are more susceptible due to anatomical factors. Symptoms include burning during urination, frequent urge to urinate, cloudy or strong-smelling urine, and lower abdominal pain. Prompt treatment with appropriate antibiotics based on culture sensitivity prevents complications.

Our Treatment Approach

We perform regular renal function monitoring for all patients with diabetes, hypertension, or other risk factors. Early-stage CKD management includes tight blood pressure control, blood sugar optimization, dietary protein guidance, and avoidance of nephrotoxic medications. UTIs are treated with culture-guided antibiotics, and preventive strategies are discussed.

Kidney health
Cholesterol

High Cholesterol & Lipid Disorders

Dyslipidemia, High Triglycerides, Atherosclerosis Prevention

Understanding Cholesterol

Cholesterol is a waxy substance found in the blood. While the body needs cholesterol to build healthy cells, high levels can increase the risk of heart disease. Dyslipidemia (abnormal lipid levels) is a major modifiable risk factor for atherosclerotic cardiovascular disease, which includes heart attacks and strokes.

Types of Cholesterol

  • LDL (Low-Density Lipoprotein): The "bad" cholesterol. High levels cause plaque buildup in arteries, narrowing and hardening them (atherosclerosis).
  • HDL (High-Density Lipoprotein): The "good" cholesterol. It helps remove LDL from arteries and transport it back to the liver for processing.
  • Triglycerides: The most common type of fat in the body. High levels combined with high LDL or low HDL significantly increase cardiovascular risk.

South Asian Risk Factor

South Asians, including Indians, have a genetic predisposition to atherogenic dyslipidemia — characterized by high triglycerides, low HDL, and small dense LDL particles. This pattern significantly increases cardiovascular risk even at seemingly normal total cholesterol levels, making aggressive lipid management essential.

Our Treatment Approach

Dr. Singh performs comprehensive lipid profiling and cardiovascular risk assessment. Treatment includes statin therapy when indicated, lifestyle modifications (dietary changes, exercise, weight management), addressing associated conditions (diabetes, hypertension), and regular monitoring to achieve target lipid levels based on individual risk profiles.
